Key Responsibilities
  • Maintain confidentiality of residents' protected health information and report any suspected violations to the appropriate authorities.
  • Engage with residents and their families during the admission process to explain policies regarding rights and advance directives.
  • Conduct thorough social services assessments to identify resident needs and challenges.
  • Coordinate and provide social work services tailored to the unique needs of residents and their families.

  • Collaborate with facility staff to address resident concerns effectively.

Qualifications
  • A high school diploma or equivalent is required.
  • Preferred: At least one year of experience in long-term care settings.
  • Ability to assess resident needs and manage discharge planning effectively.
  • Must be compassionate, dedicated, and committed to delivering high-quality care.
