Job Title: Community Health Worker MMU

Job Summary: We are seeking a highly motivated and compassionate Community Health Worker MMU to join our team at CAN Community Health. The successful candidate will be responsible for providing patient care under the direct supervision of the MMU Manager and will be assigned to our mobile medical unit.

Key Responsibilities:

  • Front desk duties for MMU, including creating patient accounts in eCW and ensuring that new patient paperwork is complete and uploaded into chart.
  • Ensuring that all documentation is compliant with professional and legal standards.
  • Performing rapid HIV, Hep C, and STI testing, and filling out paperwork and 1628s while testing.
  • Setting up a table outside the MMU and handing out swag and information.
  • Participating in outreach events outside of normal working hours.
  • Assisting in the clinic as needed and driving the small outreach unit (MTU) as needed.

Requirements:

  • High School Diploma.
  • CPR certified.
  • Valid driver's license.
  • Electronic Medical Records classes through SuccessEHS.
  • HIV classes through local health departments or on-line.

Work Environment: This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, audio visual, telephones, photocopiers, filing cabinets and fax machines.

Physical Requirements: Neat, professional appearance. Requires full range of body motion including handling and lifting patients. Exert up to 50 lbs. of force occasionally, and/or 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects, including the human body.

Travel: This position will be assigned to staff our mobile medical unit. Travel will be primarily during the business day, although some out-of-area, weekend and overnight travel may be expected.
