Role Summary:
The Patient Support Coordinator will oversee the entire patient journey, acting as a supportive ally throughout their care process. This role involves managing the patient's care plan within the Third-Party Reproduction Program to ensure optimal outcomes. Collaboration with clinical staff is essential to monitor the patient's progress and ensure compliance with established clinical standards.
Key Responsibilities:
- Welcome patients at the front desk and guide them through their appointments.
- Communicate with external practices to gather daily results and input them into our systems.
- Assist patients in obtaining medical records and prepare them for clinical review.
- Support the maintenance of regulatory files and documentation for inspections.
- Retrieve and organize medical reports for physician review.
- Act as a bridge between patients and clinical teams.
- Advocate for patients by addressing their inquiries and concerns with the appropriate staff.
- Update patient information in electronic medical records and scheduling systems.
- Encourage participation in wellness programs, including yoga, support groups, and nutrition services.
- Handle protected health information (PHI) in accordance with HIPAA regulations.
- Perform additional duties as required.
Qualifications:
- High School Diploma or equivalent.
