Job Summary

We are seeking a compassionate Registered Nurse to join our interdisciplinary team as a Hospice RN Case Manager.

Key Responsibilities:

  • Travel to patient locations, including homes, nursing facilities, and assisted living centers, to provide high-quality care.
  • Evaluate and ensure the appropriateness of patient care plans.
  • Cultivate effective communication with patients and their families to address their needs and concerns.
  • Deliver light clinical care to patients in their preferred settings.
  • Manage a caseload of approximately 15 patients, prioritizing individualized attention and support.

Requirements:

  • A valid Registered Nurse License.
  • Comfort and experience working in hospice care, preferably in emergency departments or ICUs.
