Position Summary:

The primary objective of this role is to assist families in preparing for permanent housing and to connect them with a supportive network post-housing. The focus of case management is to ensure families achieve successful rehousing placements while providing ongoing support to help them develop the necessary skills and resources for sustaining their housing.

Key Responsibilities:

  • Oversee the intake process, assessments, and data entry for all program participants, ensuring timely and accurate documentation.
  • Address and resolve tenancy-screening barriers, including assistance with securing necessary identification documents.
  • Support families in finding permanent housing by leveraging all available resources, including the private rental market and various subsidized housing options.
  • Assist families in enrolling for entitlement benefits such as SNAP, TAFDC, and SSI.
  • Collaborate effectively with community partners to enhance service delivery.
  • Ensure compliance with DHCD Shelter Scope of Services.
  • Conduct regular meetings with families to monitor progress and address concerns.
  • Keep supervisors informed of any issues or potential challenges.
  • Participate in relevant meetings and training sessions as required.
  • Perform additional duties as assigned by the Director of Shelter Programs.

Qualifications:

  • Associate's or Bachelor's degree in Human Services or a related field.
  • A minimum of two (2) years of relevant experience.
  • Proven case management abilities.
  • Strong written and verbal communication skills.
  • Excellent interpersonal skills.
  • Empathy towards diverse socio-economic populations.

Work Authorization: Must be eligible to work in the United States.
