About the Role

We are seeking a skilled Medical Care Specialist to join our team at Cornerstone Rehab. This role will involve delivering high-quality care to adult and geriatric patients, working collaboratively with a diverse team of clinical professionals.

Key Responsibilities:

  • Provide comprehensive physical therapy services to patients, tailoring treatment plans to meet their unique needs and capabilities.
  • Prepare accurate and detailed documentation, progress notes, and discharge summaries to ensure seamless communication among healthcare professionals.
  • Establish effective care plans in accordance with patients' goals and limitations, promoting optimal outcomes and quality of life.

Requirements:

  • A valid State Physical Therapy Assistant License and Credentials are mandatory for this position.
  • Previous experience in SNF and/or Acute care settings is highly desirable.

The estimated salary range for this position is $60,000 - $80,000 per year, depending on location and level of experience.
