RN Care Coordinator Job Summary

We are seeking a skilled RN Care Coordinator to join our team in Lewis County. As a RN Care Coordinator, you will play a vital role in coordinating care services for our members, working closely with the primary care team and reporting to the Clinical Services Manager.

Key Responsibilities:
  • Establish and maintain effective communication with members, caregivers, and healthcare providers to ensure seamless care coordination.
  • Review member status and adjust care plans as necessary to achieve progress toward goals.
  • Perform administrative tasks, such as data collection and entry, to support care coordination efforts.
  • Participate in program effectiveness analysis and planning to ensure high-quality care.
  • Visit members in their homes or in patient settings to provide care coordination and support.
  • Communicate with members, caregivers, and healthcare providers to ensure timely and effective care.
  • Assist with other clinical and quality responsibilities, such as identifying gaps in care.
Requirements:
  • Current unrestricted Registered Nurse license in Washington state.
  • 3+ years of RN experience, with 1+ year of case management experience.
  • Experience with electronic medical records and basic Microsoft Office skills.
  • Proven excellent communication and customer service skills.
  • Valid driver's license and ability to travel locally 75% of the time.
Preferred Qualifications:
  • BSN degree.
  • Experience working in post-acute care, such as long-term care or hospice, and/or case management experience as an RN.
  • Experience working with the geriatric population.
  • Familiarity with the long-term care business environment.
