About the Opportunity

Cape Cod Healthcare is seeking a skilled and compassionate Behavioral Health Specialist to join our team as a Mental Health Services Coordinator. As a key member of our healthcare team, you will be responsible for providing high-quality care and support to patients and their families.



Job Summary

The Behavioral Health Specialist - Mental Health Services Coordinator will work closely with multidisciplinary teams to assess and drive treatment planning during acute admission and transition to discharge. This role requires strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.



Key Responsibilities

Some of the key responsibilities of this role include:



  • Conducting initial clinical assessments of individuals presenting with acute care needs, in accordance with Department of Mental Health state requirements;
  • Preparing and communicating initial treatment plans to multidisciplinary teams;
  • Leading treatment and discharge planning in collaboration with psychiatrists and multidisciplinary treatment teams;
  • Providing group therapy, individual and family counseling, and other related group sessions;
  • Assisting in patient care planning to ensure continuation of treatment in a supportive and health-promoting environment;
  • Evaluating ongoing medical and psychosocial needs for home care planning and setting up appropriate community services;
  • Linking patients to available programs and services to provide maximum benefit while in the hospital and as a continuum after discharge;
  • Assisting patients and families with the procurement of essential equipment for safe and optimal functioning at home;
  • Assisting with decision-making processes leading to the selection and placement of patients in appropriate facilities, if needed;
  • Bringing voluntary community-based services into play as appropriate to patient and family needs;
  • Functioning as a liaison on behalf of patients with community agencies;
  • Evaluating the appropriateness of community services to develop plans to meet patient needs;
  • Initiating crisis prevention and crisis intervention services as needed, in coordination with patient families and primary caregivers;
  • Providing timely communication with managed care companies for approval of required levels of care;
  • Maintaining knowledge of relevant regulatory agency standards, including CMS, DPH, DMH, and others.


Requirements

To be successful in this role, you will need:



  • Ability to read, write, and communicate effectively in English;
  • Massachusetts licensure as a Licensed Independent Clinical Social Worker (LICSW) or other specified licenses;
  • Crisis Prevention Intervention Training within 1 month of new position;
  • BLS Certification within 3 months of new position and ongoing maintenance.


Compensation and Benefits

The estimated salary for this role is $85,000 - $110,000 per year, depending on experience and qualifications. We also offer a comprehensive benefits package, including health insurance, retirement savings, and paid time off.
