Job Summary

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and develop personalized treatment plans to improve their mobility, endurance, and overall well-being.
  • Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop targeted interventions to address their specific needs.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Provide education and support to patients and their families to promote healthy habits and independence.
Requirements
  • Current Physical Therapy licensure in the state of New Jersey.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
