About the Role

As a Home Health Physical Therapist, you will work with patients in the comfort of their own homes, providing personalized care and support to help them achieve their goals and improve their quality of life.

Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify areas for improvement
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to help patients achieve optimal mobility and function
  • Collaborate with nursing and other healthcare professionals to ensure comprehensive care
Requirements
  • Active and current license to practice as a professional Physical Therapist in the state of New Jersey
  • Previous home healthcare experience welcomed, but not required
  • Ability to travel to cases as assigned
