Job Title: Home Health Physical Therapy Assistant

We are seeking a skilled Home Health Physical Therapy Assistant to join our team at LHC Group. As a Home Health Physical Therapy Assistant, you will play a vital role in providing high-quality care to patients in their homes.

Job Summary:

The Home Health Physical Therapy Assistant will work under the supervision of a licensed Physical Therapist to provide physical therapy services to patients in their homes. The ideal candidate will have excellent communication skills, be able to work independently, and have a strong commitment to providing exceptional patient care.

Responsibilities:
  • Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions as prescribed by the Physical Therapist.
  • Assist patients with daily living activities, such as bathing, dressing, and grooming.
  • Monitor patients' progress and report any changes or concerns to the Physical Therapist.
  • Collaborate with other healthcare professionals to ensure comprehensive care for patients.
  • Document patient progress and maintain accurate records.
Requirements:
  • Current licensure as a Physical Therapy Assistant in the state of New Jersey.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift and move patients as needed.
