Central City Concern is seeking a skilled Mental Health Counselor to join our Intensive Case Management Team. As a key member of our multidisciplinary team, you will provide clinical care to clients enrolled at Old Town Recovery Center, working closely with a care team to ensure comprehensive, proactive, and client-centered care.
Key Responsibilities- Deliver care to clients assigned to the team, ensuring accountability, comprehensiveness, and coordination.
- Conduct mental health assessments, including mental status exams, DSM diagnoses, and clinical formulations.
- Develop and implement treatment plans with clients, collaborating with the care team to support recovery and health goals.
- Interface and collaborate with the ICM team to facilitate individual treatment plans and coordinate residential and non-residential treatment services.
- Participate in clinical staff meetings and case conferences to utilize interdisciplinary expertise and facilitate treatment planning.
- Master's degree in a human services field and one year of experience in mental health.
- Registration with MHACBO as a QMHP-R or QMHA-R.
- Ability to work effectively within a multidisciplinary team and in an environment subject to ambiguity and change.
- Knowledge of community and social service agencies, mental health treatment methods, and crisis intervention.
- Ability to communicate clearly and concisely, both orally and in writing.
