Case Manager

LHH is seeking a compassionate and experienced Case Manager to join our team in Los Angeles, CA. As a Case Manager, you will play a critical role in providing support and resources to individuals experiencing homelessness, with the ultimate goal of improving their quality of life and securing stable housing.

Key Responsibilities:

  • Conduct thorough assessments with clients to identify their needs and develop personalized action plans.
  • Connect clients with local resources and services to achieve housing stability.
  • Maintain regular contact with clients to monitor their progress and provide ongoing support.
  • Advocate on behalf of clients to ensure their rights and needs are met.

Requirements:

  • At least one year of experience working with the homeless population, preferably in a social services or case management role.
  • Strong communication and interpersonal skills.
  • Familiarity with housing systems and Fair Housing laws is a plus.
