About the Role

The Nurse Practitioner at Abode Care Partners will be responsible for examining and treating illnesses, ordering diagnostic tests, and collaborating with healthcare professionals to maintain a comprehensive care plan.

Responsibilities
  • Provide evidence-based medical care to patients, including examination, diagnosis, and treatment of chronic and acute conditions
  • Order and interpret laboratory and diagnostic testing as needed
  • Collaborate with healthcare professionals to develop and implement individualized care plans
  • Ensure accurate and timely documentation of patient medical records
Requirements
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
