Job Summary

Key Responsibilities
  • Appointment Scheduling: Coordinate and schedule medical and behavioral health appointments for adults and those working with the Prevention Case Manager.
  • Patient Tracking: Track and monitor the therapist's and Prevention Case Manager's patients.
  • Task Management: Monitor tasks and orders of assigned providers.
  • Record Review: Complete a full record review of Adults sent to the behavioral health pool.
  • Workflow Management: Follow the most current work-flow of the Patient Care Coordinator.
  • Quality Patient Care: Perform related duties necessary to ensure quality patient care as requested by supervisor, including addressing barriers when the patient has not met treatment goals.
  • Task Processing: Process all tasks and orders, including phone calls from patients, routing as appropriate and responding in a timely manner.
  • Document Management: Scan and fax documents to be read, reviewed, placed in appropriate charts and/or routed as appropriate daily.
  • Referral Coordination: Coordinate, track and monitor outside referrals (e.g. Direct Secure Message, obtain outside specialty records).
  • Service Linkage: Facilitate linkage to services to mitigate Social Determinants of Health (e.g. MLP, CSS, Food Pantry).
  • Prevention Case Manager Support: Support the work of the Prevention Case Manager; assist in care coordination and follow-up.
  • Regulatory Compliance: Demonstrate knowledge of and compliance with regulatory agencies and established organizational policies and procedures.
  • Follow-up Scheduling: Schedule patients for follow-up appointments based on established protocol, identified need and availability of appointments; follow-up with patients who have to keep important appointments.
  • Customer Service: Exhibit customer service consistent with organizational standards.
  • Communication: Openly, clearly and respectfully share and receive information, opinions and concerns and feedback in a supportive manner.
  • Team Collaboration: Work collaboratively by mentoring new and existing co-workers, building bridges and creating rapport with team members across the organization.
Requirements
  • Education: Minimum of High School Diploma or GED.
  • Experience: 2 years professional experience in a health or behavioral health practice preferred.
