Center Medical Director

At Oak Street Health, we're on a mission to rebuild healthcare as it should be. We're seeking a highly skilled and experienced Center Medical Director to lead our clinical team at our Wilmington Center.
About the Role:The Center Medical Director will be responsible for leading the clinical team in providing high-quality patient care and creating a positive work environment. This role requires strong leadership and clinical skills, as well as the ability to work collaboratively with our practice manager and outreach manager/director to achieve our organizational objectives.
Key Responsibilities:- Support the implementation of our organizational strategy and drive clinical excellence
- Lead the onboarding of new providers and model excellent medical care
- Ensure that care teams are effectively preparing for and executing daily huddles, patient care planning and management, admission prevention work, and clinical documentation workflows
- Understand, teach, and implement Oak Street's clinical guidelines at both the individual provider and center level
- Support each care team and the center to provide the highest quality preventative care
- Assess and address root causes when center metric goals are not reached
- Lead and coach providers to be the best they can be for their patients and colleagues
- M.D., D.O., or A.P.N or P.A. graduates
- Board certification in Internal Medicine or Family Medicine required for M.D. or D.O., Board certification in Family, Adult, or Gerontology for A.P.N
- Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and colleagues
- Experience using a metrics-driven approach to the provision of medical care and/or quality projects
- Passionate about their work and motivated to be a part of a fast-growing organization
