Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a highly skilled and compassionate Home Health Physical Therapist to join our team in Northfield Falls, Vermont.

Job Summary:

We are looking for a dedicated and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote the health, safety, and independence of our clients.

Responsibilities:

  • Conduct home visits to assess and evaluate clients' physical therapy needs
  • Develop and implement individualized care plans that address clients' physical therapy needs
  • Provide manual therapeutic exercises, gait training, balance, and other interventions as needed
  • Continually assess and revise care plans to ensure optimal client outcomes
  • Collaborate with nursing and other healthcare professionals to ensure comprehensive care

Qualifications:

  • Current Vermont Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
