Job Title: Home Health Physical Therapy Assistant

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapy Assistant to join our team in Northfield, New Jersey.

Job Summary:

We are looking for a dedicated and experienced Physical Therapist Assistant to provide high-quality care to our adult and geriatric clients in their homes. As a Home Health Physical Therapy Assistant, you will work closely with our team to develop and implement personalized care plans that meet the unique needs of each client.

Responsibilities:
  • Provide physical therapy services to clients in their homes, including exercises, stretches, and other interventions to improve mobility and function.
  • Assess clients' physical abilities and develop individualized care plans to meet their needs.
  • Work closely with our team to coordinate care and ensure seamless transitions between services.
  • Communicate effectively with clients, families, and healthcare professionals to ensure optimal care.
  • Document client progress and outcomes in a timely and accurate manner.
Requirements:
  • Current licensure as a Physical Therapist Assistant in New Jersey.
  • Minimum of one year of recent, verifiable clinical experience.
  • Prior home care experience is strongly preferred, but not required.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills and ability to communicate effectively with clients, families, and healthcare professionals.
