Role Summary:
We are seeking a Community Outreach Specialist with a robust background in community engagement or a strong commitment to promoting health equity and social justice for underserved populations. This role is essential to our community impact strategy, requiring the right candidate to possess experience in community outreach and a desire to serve with empathy and cultural competence. Availability for ad hoc outreach activities is essential.

Key Responsibilities:

  • Be available for last-minute Home Visits / Community Engagement events.
  • Conduct in-person home visits within assigned communities and perform assessments or enrollments based on participant needs (visits may occur during various times, including evenings and weekends).
  • Provide timely resources and information regarding available community services.
  • Respond sensitively and competently to the cultural and socio-economic characteristics of the service population.
  • Complete daily data entry and necessary documentation promptly.
  • Consistently achieve goals established by leadership.
  • Participate in team meetings on a daily, weekly, and monthly basis.
  • Undertake additional tasks as assigned by leadership.

Essential Qualifications:

  • Available for last-minute Home & Hospital Visits.
  • Willing to travel 1-2 hours for visits (travel time compensated).
  • Associate's degree or equivalent professional experience.
  • Preferred: 2+ years of experience in community outreach.
  • Flexible schedule to accommodate visits during various times.
  • Must possess a vehicle, valid driver's license, and proof of insurance.
  • Proficient computer skills and adaptability to new technologies.
  • Strong time management abilities.
  • Excellent verbal and written communication skills.
  • Bilingual proficiency is a significant advantage.
  • Must pass a pre-employment background check and drug screening.
  • Ability to work independently and manage time effectively.
Compensation & Benefits
