Job Summary

We are seeking a skilled Nurse Practitioner to join our team at Resources For Human Development. As a Nurse Practitioner, you will provide high-quality primary care services to patients of all ages, from infants to elderly. Your expertise will include medical assessment, diagnosis, treatment, referral, and follow-up care.

Key Responsibilities

  • Provide comprehensive primary care services to patients, including medical assessment, diagnosis, treatment, referral, and follow-up care.
  • Manage acute and chronic conditions, both simple and complex, using evidence-based criteria and established clinical practice guidelines.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and health care needs.
  • Document patient care in a timely and accurate manner, meeting acute care compliance standards.
  • Consult with collaborating physicians and other healthcare team members as needed.
  • Develop and implement individualized plans of care to address patients' health care needs.
  • Prescribe diagnostic strategies and therapeutic interventions as needed.
  • Meet productivity standards and established goals.
  • Perform advanced procedures consistent with privileges and competency validation.
  • Participate in meetings, mandatory in-services, educational programs, and other activities to stay informed and current.
  • Maintain required continuing education credits as defined by specific governing bodies associated with licensure.
  • Knowledgeable and supportive of policies, procedures, and guidelines associated with practice setting.

Requirements

  • Three to five years of prior experience as an RN and/or Nurse Practitioner.
  • Master's degree from an accredited nurse practitioner program.
  • Current RN and CRNP licenses from the Commonwealth of Pennsylvania.
  • Current nationally recognized advanced practice certification or in progress.
  • Physical and emotional ability to perform duties and responsibilities associated with the position.
  • Proficiency in basic computer skills.
  • Understanding of psychosocial impacts of health outcomes in underserved communities.
  • Exhibits cultural competency and sensitivity.
  • Supports FPCN's commitment to the creation of a trauma-informed system of care.
