Nurse Practitioner Position

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Resources For Human Development. As a key member of our healthcare team, you will provide high-quality primary care services to patients of all ages, from infants to elderly.
Responsibilities- Provide a wide range of primary care services, including medical assessment, diagnosis, treatment, referral, and follow-up.
- Manage acute and chronic conditions, both simple and complex, using evidence-based criteria and established clinical practice guidelines.
- Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and healthcare needs.
- Document patient information in a timely and accurate manner, meeting acute care compliance standards.
- Consult with collaborating physicians and other healthcare team members as needed.
- Develop and implement individualized plans of care to address patients' healthcare needs.
- Prescribe diagnostic strategies and therapeutic interventions as necessary.
- Meet productivity standards and established goals.
- Perform advanced procedures consistent with privileges and competency validation.
- Participate in meetings, mandatory in-services, educational programs, and other activities to stay informed and current.
- Maintain required continuing education credits.
- Stay knowledgeable about policies, procedures, and guidelines associated with the practice setting.
- Three to five years of prior experience as an RN or Nurse Practitioner.
- Master's degree from an accredited nurse practitioner program.
- Current RN and CRNP licenses from the Commonwealth of Pennsylvania.
- Current nationally recognized advanced practice certification or in progress.
- Physical and emotional ability to perform the duties and responsibilities associated with the position.
- Proficiency in basic computer skills.
- Understanding of psychosocial impacts of health outcomes in underserved communities.
- Exhibits cultural competency and sensitivity.
- Supports FPCN's commitment to creating a trauma-informed system of care.
- Demonstrates cultural competence and proficiency in interactions with others.
- Ability to build and maintain positive and professional relationships.
- Ability to create a safe and supportive environment for staff and patients.
- Ability to support individuals on their paths to recovery and healing.
- Demonstrates exemplary problem-solving, communication, interpersonal, and conflict resolution skills.
- Ability to work effectively as a team member.
- Highly responsible and reliable, with a courteous demeanor.
- Unconditional ability to maintain patient confidentiality.
- Exemplary organizational skills and ability to prioritize.
- COVID vaccination required.
- CPR certification required.
