Job Summary
We are seeking an experienced MDS Coordinator to join our team, responsible for overseeing the Resident Assessment Instrument (RAI) process, conducting assessments, and coordinating care plans for our guests. The ideal candidate will have a strong background in nursing and experience working with the RAI process, including the Prospective Payment Process (PPS).
About the Role
As Senior Clinical Care Coordinator, you will be responsible for:
- Completing the MDS, CAA's, and care plans within regulated time frames
- Coordinating scheduling the RAI process with the interdisciplinary team
- Assessing residents through physical assessment, interview, and chart review
- Discussing resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
- Reviewing information from hospital, consults, and outside agencies and using such information in the completion of the assessment and care planning
- Coordinating, identifying, and/or initiating significant change MDS'
- Maintaining PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
- Remaining current with American Association of Nursing Assessment Coordinators (AANAC) requirements
Requirements
To be successful in this role, you will need to have:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
- AANC certification a plus. RAC-CT
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred
- Experience as an MDS Nurse
