We are seeking a skilled Pharmacist to join our team at Harbour View Medical Center. As a Pharmacist, you will play a critical role in ensuring the safe and effective use of medications for our patients.
Key Responsibilities- Conduct initial and ongoing patient assessments, analyzing data to create a plan of care and implementing treatment.
- Administer medications in a safe manner, consistent with state practice and Bon Secours Mercy Health policies.
- Serve as a point of contact for patients' care coordination throughout hospital departments.
- Act as a patient safety advocate, participating in ongoing quality improvement in the department.
- Process orders and provide medication review for appropriateness, safety, and efficacy.
- Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD.
- Advanced pharmacy degree (Doctor of Pharmacy, Masters, Residency) preferred.
- Maintains current license for pharmacist in the state(s) where practicing.
- Valid state-issued ID.
- BLS Basic Life Support (preferred).
- Immunization Certified (preferred).
- 2 years of recent experience in an acute care setting.
- 1 year of leadership experience (preferred).
