Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Clearway Health. As a Clinical Pharmacist, you will play a critical role in optimizing patient outcomes related to chronic and specialty drug-related disease states.

Key Responsibilities
  • Collaborate with referring physicians to optimize pharmacotherapy according to standard care guidelines for optimal patient outcomes.
  • Provide direct patient care in an ambulatory setting, focusing on managing chronic and specialty drug-related disease states.
  • Evaluate patient response to medication therapy based on laboratory values, patient reported outcomes and optimize pharmacotherapy based on clinical assessment of findings.
  • Provide disease state and medication education to patients and care team.
  • Assist in managing medication-related problems in clinics and act as the drug information resource.
  • Serve as a liaison between outpatient pharmacies and clinic staff, aiding in resolving third-party coverage barriers.
  • Document patient interactions in the electronic medical record.
  • Identify and implement cost-saving and containment programs.
  • Participate in adverse drug reaction and medication error reporting programs.
  • Represent the department in hospital committees and attend staff development activities.
  • Maintain professional competency through continued education.
  • Develop fiscal year goals aligned with institutional objectives.
  • Support 340b program operations, including patient identification and staff education.
  • Assist with prescription intake, filling, or delivery on a limited basis.
Requirements
  • A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
  • Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
  • Completion of a PGY-2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship) is preferred.
  • An active Pharmacist licensure (in good standing) to practice pharmacy in the state in which you are applying in or and/or the ability to acquire this at the time of hire is required.
  • Must possess or be able to obtain a State Board of Pharmacy Preceptor's license by the current state you are applying to within one year of hire.
  • 2+ years of work experience in ambulatory care pharmacy setting post PGY1 training or 3+ years post PharmD is preferred.
Preferred Qualifications
  • Demonstrated expertise in medications, drug interactions, medical procedures, therapies, diagnoses, and disease states is essential.
  • Exceptional interpersonal skills for effective communication with diverse stakeholders including hospital staff, patients, families, physicians, and colleagues.
  • Ability to effectively manage multiple tasks, prioritize duties, and adhere to deadlines.
  • Organized work approach with a proficiency in maintaining accurate documentation.
  • Strong verbal and written communication skills; proficiency in additional languages is advantageous.
  • Self-directed with strong self-management abilities; capable of identifying and addressing issues independently.
  • Proven track record in process improvement and a keen eye for detail and precision.
  • Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers.
Work Environment

This position will be located on site at our client partner, East Boston, a community health center that opened in 1970 and has since operated 24 hours a day, 7 days a week.

Today they are the only health center in New England, and one of the few in the country to provide continuous care.

Their services and development have been defined by the needs of the communities they serve.

They provide a comprehensive system of services including primary and specialty care, a 24-hour Satellite Emergency Facility, two on-site pharmacies, and the comprehensive Neighborhood PACE plan for older adults, as well as innovative programs to help address the health and well-being of community members with specific needs, including HIV/AIDS, chronic diseases, children with multiple disabilities and childhood obesity.

They are the largest community health center in Massachusetts and among the largest community health centers in the country.
