Advanced Triage Nurse

We are seeking an experienced Advanced Triage Nurse to join our team at PCHC, a dynamic urban community health center. In this role, you will play a vital part in providing high-quality patient care and contributing to the success of our organization.
**Job Summary:**
The Advanced Triage Nurse will be responsible for receiving, triaging, and responding to incoming telephone calls from patients seeking clinical direction and advice. This involves quickly and accurately assessing patients' needs by actively listening to callers and asking proactive questions. The successful candidate will have excellent assessment, critical thinking, and communication skills, with the ability to work effectively in a fast-paced medical environment.
**Key Responsibilities:**
- Receive and respond to incoming telephone calls from patients seeking clinical direction and advice
- Use standardized adult and pediatric protocols to assess patients' needs
- Direct patients to the appropriate level of care, ensuring they are correctly scheduled with a PCHC provider when necessary
- Identify emergent situations and effectively triage to higher levels of care
- Makes clinically appropriate recommendations for action/intervention and issue resolution
**Requirements:**
- Licensed Registered Nurse (RN) in good standing without restriction in the state of Rhode Island
- Minimum of 5 years nursing experience in an emergency room setting, primary care, or urgent care clinic
- Strong nursing assessment skills and level of comfort working in a fast-paced medical environment, both independently and as a member of the medical team
- Experience in Triage, or similar ability to rapidly use the nursing process to assess, diagnose, plan, implement, and evaluate in real-time situations
- Comfortable communicating with patients using interpreters
- BLS Certification required
