Job Summary

We are seeking a highly skilled and compassionate In-Home Services Coordinator to join our team at South Coast Business Employment Corporation. As a key member of our staff, you will play a vital role in ensuring the well-being and independence of seniors and individuals with disabilities in Coos County.

Key Responsibilities
  • Case Management: Assess participants for Older Americans Act and Oregon Project Independence program eligibility through home visits, evaluating their Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Prepare service plans to meet the needs of aging and disabled adult clients, referring them to outside agencies and guiding them through agency procedures.
  • Coordination: Provide service coordination for adults with chronic or acute age-related or physical disabilities, determining eligibility for services and developing care plans to prevent premature institutionalization. Coordinate referrals to other community services and monitor clients' progress.
  • Program Support: Complete and review application forms for home-delivered meal eligibility, provide program explanations to participants, and gather required data. Maintain computerized agency program/fiscal performance reports and compile data for all services provided through the Area Agency on Aging.
Requirements
  • Knowledge and Skills: Knowledge of local, state, and federal laws and regulations relevant to the program area, as well as basic understanding of the aging process and its impact on the older adult population.
  • Ability to: Organize and maintain computer data and reports, demonstrate proficiency in oral and written communication, and establish and maintain effective working relationships with personnel of all programs within the AAA.
