Job Summary

We are seeking a highly motivated and compassionate Community Outreach Specialist to join our team at Grand Lake Mental Health Center. The successful candidate will be responsible for conducting outreach to high-risk and homeless individuals, educating them about HIV, HEP C, STDs, and overdose prevention tools. This is a full-time position that requires a strong understanding of the target population and the ability to blend in with the community served.

Key Responsibilities
  • Conduct outreach to locate, engage, and assist high-risk and homeless individuals with access to necessary substance use and behavioral health services.
  • Document all contacts and encounters with high-risk and homeless individuals on a daily basis.
  • Conduct HIV/HEP C rapid testing in high-risk groups during street and community outreach efforts.
  • Ensure all test results are provided to individuals and secured to ensure confidentiality.
  • Enter all data into the Public Health Investigation and Disease Detection of Oklahoma (PHIDDO) system.
Requirements
  • Certification of either PRSS or Case Manager required.
  • At least one year of experience in working with alcoholics/addicts in a recognized treatment or rehabilitation program.
  • Two years of employment or volunteer work in the drug and alcohol field preferred.
Preferred Qualifications
  • Knowledge of the problems of homelessness in relation to alcoholism/addiction and co-occurring disorders.
  • Skills in interviewing and crisis intervention.
  • Establishing and maintaining working relationships with others.
