Liberty Medical Care Services is a leading provider of quality healthcare services. We are currently seeking an experienced Nurse Practitioner to join our team.

Compensation: $10,000 Sign on Bonus and estimated annual salary of approximately $120,000

Job Description:
  • Deliver high-quality patient care in local Skilled Nursing & Rehabilitation facilities, ensuring the highest standards of nursing practice.
  • Direct and coordinate nursing care to assure quality outcomes for patients, focusing on early identification of changes in condition and effective management of chronic conditions.
  • Conduct thorough physical assessments and annual evaluations for assigned patients, identifying potential health risks and developing personalized plans of care.
  • Complete acute visits to assess and diagnose patient conditions, providing timely interventions and referrals as needed.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Prepare discharge orders, instructions, and prescriptions for patients being discharged, ensuring a smooth transition to post-acute care settings.
  • Encourage advanced care planning and end-of-life decisions for patients requiring these services, respecting their autonomy and preferences.
  • Provide pharmacy with necessary narcotic prescription refills as patient need dictates, adhering to established protocols and guidelines.
  • Document patient assessment and plan of care in the electronic medical record, maintaining accurate and up-to-date records.
  • Review labs and other diagnostic tests for additional interventions, using clinical judgment to inform treatment decisions.
  • Communicate daily with nursing staff, assigning duties, supervising, and evaluating work performance to ensure efficient and effective care delivery.
  • Stay up-to-date with nursing policies and procedures, making recommendations for changes or additions as necessary to ensure compliance and best practices.
  • Develop and conduct specific areas of in-service or continuing education, enhancing the knowledge and skills of nursing staff.
  • Assess patients for palliative care needs and create/oversee the palliative plan of care in facilities and patient homes, prioritizing comfort and dignity.
Requirements:
  • Current, unrestricted valid Nurse Practitioner license from the Board of Nursing or the ability to obtain.
  • DEA certificate current or the ability to obtain.
  • Clinical background in adult, family, or geriatric specialties preferred.
  • Past experience working in long-term care, a nursing home, or with seniors in other settings preferred.
  • Strong communication and relationship-building skills preferred.
  • Intermediate level of proficiency in PC-based word processing, including Microsoft Word and Outlook.
  • Willingness to be 'on call' for facilities as needed.
  • Dependability, flexibility, and ability to work well with all nursing personnel and other departments.
  • Ability to make decisions regarding nursing problems, work well under pressure, and problem-solve effectively.
