Key Responsibilities- Provide comprehensive wound care services, including dressing changes, debridement, and minor procedures, to patients in SNFs.
- Conduct thorough assessments and develop individualized care plans to address the unique needs of each patient.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Stay up-to-date with the latest wound care best practices and guidelines to provide evidence-based care.
- Document patient interactions, treatments, and outcomes in a timely and accurate manner.
- Current state license as a Nurse Practitioner.
- DEA license.
- At least 1 year of experience as a Nurse Practitioner, preferably in a post-acute care or wound care setting.
- Experience with minor procedures, such as suturing and central line placement, is highly desirable.
