At Life Care Center of Farmington, we are seeking a skilled Registered Nurse to join our team as a Patient Care Coordinator.

As a RN Supervisor, you will be responsible for coordinating patient care and services. Your expertise in promoting quality care using a collaborative process will ensure the highest level of physical, mental, and psychosocial well-being for our patients.

Job Summary:

We offer an estimated salary of $80,000 - $110,000 per year, based on experience and qualifications. This role requires a strong foundation in nursing principles, with a focus on leadership and coordination of patient care.

About the Role:

  • Clinical Expertise: As a RN Supervisor, you will utilize your extensive knowledge in geriatric nursing to develop and implement effective care plans for patients.
  • Leadership Skills: You will lead a team of licensed staff and CNAs in prioritizing patient care needs and ensuring seamless communication among team members.
  • Patient Advocacy: You will work closely with patients and their families to provide excellent customer service and promote a positive attitude towards care.

Responsibilities:

  • Evaluate quality of patient care rendered
  • Develop and implement patient care plans
  • Maintain accurate records and reports
  • Communicate effectively with interdisciplinary teams

Requirements:

  • Associate's or Bachelor's degree in Nursing from an accredited college or university
  • Current licensure/registration in applicable State
  • Two (2) years' nursing experience, with geriatric nursing experience preferred
  • CPR certification upon hire or obtain during orientation
