Job Title: Services Coordinator Supervisor

The Department of Developmental Services (DDS) is seeking a highly skilled and experienced Services Coordinator Supervisor to join our team in the Southeast Region at the Cape & Islands Area Office. As a key member of our team, you will provide direct supervision to 6-8 Human Service Coordinator I (HSC I) staff, including assigning cases, providing support for crisis management, conflict resolution, and facilitating Individual Service Plan (ISP) development.

Key Responsibilities:
  • Ensure each component of the ISP process and/or contracted services are completed for each individual receiving support from the Department.
  • Respond to urgent requests and complete routine referrals to area programs and services for individuals.
  • Provide support and consultation to local area provider agencies, family members, and Service Coordinators to facilitate cooperative communication and resolution to critical issues.
  • Participate in provider meetings, planning for services, and sit on the Area Office Management Team and Risk Management Committee.
  • Work with the Area Director and Assistant Area Director to maintain automated data systems, including compiling specific statistics as needed, and managing related projects within the Area Office.
Preferred Qualifications:
  • Experience working with individuals with intellectual disabilities, families, guardians, providers, and other stakeholders within the DDS community.
  • Good interpersonal skills.
  • Good computer skills and familiarity with the Meditech program and the Home and Community Services Information System (HCSIS).
  • Competency in basic computer skills (word processing, data base management, and e-mail).
  • Strong supervisory experience in a clinical capacity.
Agency Requirements:
  • Must be at least 18 years of age.
  • Must possess a current and valid Class D Motor Vehicle Operator's License.
Schedule Information:
  • Full-time hour weekly position - 1st shift (Day Shift).
  • Monday through Friday, 9:00 am to 5:00 pm.
  • Days off - Saturday and Sunday.

ALL EMPLOYEES ARE PAID ON A BI-WEEKLY BASIS AND MUST HAVE DIRECT DEPOSIT.

This is a Civil Service position. If there is no Civil Service list for this title, employees appointed must take and pass the next Civil Service examination when administered.

Qualifications:

First consideration will be given to those applicants that apply within the first 14 days.

Applicants must have (A) at least four (4) years of full-time, or equivalent part-time, case management or clinical/therapeutic experience (B) any equivalent combination of the required experience and the substitutions below.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Substitutions:

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.

Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required.
