About the Role

We are seeking a skilled RN Care Manager to join our Family Medicine Clinic team at Presbyterian Healthcare Services.

Key Responsibilities
  • Proactively manage patient care along a continuum by facilitating collaborative care between patients, families, physicians, and other healthcare professionals.
  • Develop and implement individualized care plans to ensure optimal patient outcomes.
  • Collaborate with interdisciplinary teams to coordinate patient care and services.
Requirements
  • Registered Nurse (RN) licensure in the State of New Mexico or Compact.
  • 1-3 years of ambulatory experience.
  • Associate Degree in Nursing.
