Family Preservation Case Manager

Kids Central, Inc. is seeking a highly motivated and experienced Family Preservation Case Manager to join our team. As a Family Preservation Case Manager, you will be responsible for providing case management and support services to families in Circuit 5, working in a team approach with the Family Preservation Specialist.
Key Responsibilities:- Provide direct services to children and their families, including reviews of comprehensive assessments, treatment plans, and contracts for services.
- Establish goal setting with families and manage and ameliorate crises.
- Participate in discharge planning, including plans for follow-up treatment and connection to community services and programs.
- Provide case management services and coordinate systems of care.
- Bachelor's Degree in the human services field or BSW required.
- Three years' experience in a comparable position in a human services agency preferred.
- Current Background Clearance Screening Letter.
- Valid Florida driver's license.
- Proof of vehicle insurance.
- Excellent oral and written skills and ability to communicate comfortably with children, parents, and community agencies/personnel.
- Organized and able to work independently with minimal supervision.
- Flexible and able to multi-task.
- Excellent working knowledge of automated systems.
This job regularly operates in the field (homes, community agencies) and in a professional office environment. This job consists of regularly driving your personal vehicle.
