Nurse Practitioner-Physician Assistant, Hospice Care Specialist

At Olesky, we are seeking an experienced Nurse Practitioner to join our Hospice/Palliative Care team in Florida.
The APRN will work in collaboration with a physician and other palliative care team members to deliver palliative care and related healthcare services to patients and families in various settings.
Key Responsibilities:
Perform medical examinations and evaluations, diagnoses, treatment, follow-up, consultation, and education for patients and families.
Assist with diagnosis, treatment, and management of acute and chronic health conditions.
Order, perform, and interpret laboratory and radiology tests within the scope of professional practice.
Write orders pertaining to informed consents, diagnostic procedures, and treatments.
Prescribe medications and other treatments.
Administer medications as prescribed.
Collaboration and Communication:
Consult with palliative care physician or designee as needed, inform attending physician of services provided, and collaborate with other physicians as needed.
Facilitate clarification of patient and family goals of care.
Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement of services provided.
Execute physicians' orders in accordance with legal, ethical, and organizational standards.
Comply with applicable laws and regulations.
Initiate reimbursement for services rendered on a timely basis.
Ensure provision of informed consent for CPR and DNR.
Recommend medications as required to treat specific populations.
Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.
Provide educational sessions to advance palliative medicine among internal staff, medical community, and community as a whole.
Participate in on-call coverage and assist in establishing schedule.
Participate in regular interdisciplinary team meetings to review active cases.
Requirements:
Experienced Nurse Practitioner with expertise in palliative care.
Strong collaboration and communication skills.
Ability to work in a fast-paced environment and prioritize tasks effectively.
Strong analytical and problem-solving skills.
Ability to maintain accurate and detailed patient records.
Strong commitment to confidentiality and patient privacy.
Ability to work effectively in a team environment.
Strong educational background in palliative care and related fields.
