About the Opportunity

We are seeking a highly skilled and experienced Clinical Resource RN to join our team at Pennant Services. As a Clinical Resource RN, you will play a critical role in supporting local clinical leaders at agencies located throughout the northeast. Your primary responsibility will be to design, monitor, implement, and evaluate processes and programs to meet regulatory and accreditation requirements and to achieve the highest standard of care.

Key Responsibilities
  • Assist with the transition to and implementation of HCHB.
  • Provide leadership and guidance in the establishment of programs, policies, and practices that ensure exceptional clinical care in compliance with State and Federal Regulatory requirements.
  • Educate and assist local clinical leaders to always be survey ready, working closely with compliance resources to ensure appropriate and effective survey preparedness.
  • Support local leadership in developing and implementing processes to monitor and measure all activities related to clinical, education, and performance improvement.
  • Provide support regarding the direction and coordination of educational programs, orientation, new hire competency systems, annual skills competency, and ongoing in-service educational program development.
  • Monitor the organization's Outcome-Based Quality Initiative (OBQI) outcomes, providing feedback and best practice education to staff to improve outcomes.
  • Coordinate activities related to clinical record review for home health and hospice data collection, compiling, trending, and reporting quality data in areas such as patient care, safety, risk management, infection control, outcomes, and customer satisfaction.
  • Educate and train nursing supervisors to ensure documentation is complete and consistent with legal requirements, and may be asked to step into patient homes for observations, evaluations, and mock surveys.
  • Help develop initiatives and monitor agency process and progress in areas including patient safety, risk management, infection control programs.
Requirements
  • Current Licensed Registered Nurse
  • CPR Certified
  • A minimum of five years' Home Health and/or Hospice experience preferred
  • Will be required to show proof of a valid driver's license, motor vehicle insurance, and a 2-step TB screen (given within the past 6 months of date of hire)
  • Understand the legal and regulatory framework governing home health/hospice industry
  • Ability to work with all levels of management on all aspects of clinical operations
  • Verbal and written communication skills necessary to explain complex and/or confidential information
  • Time management and organizational skills necessary to manage multiple projects, appropriately prioritize workload, plan for agency leaders to meet deadlines and goals, and work independently in responding to day-to-day functional needs
  • Demonstrated ability to interact effectively with employees
  • Ability to anticipate and identify problems and use sound judgment and fact-based analysis to develop effective and efficient solutions
  • Software skills, including use of Microsoft Office software and web-based applications
