About the Role:- Provide comprehensive nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Coordinate total care plans with other healthcare professionals involved in patient care and ensure continuity of care by exchanging information with physicians, agency personnel, patients, families, and community resources.
- Make initial and comprehensive nursing evaluations, determine primary focus of care, and develop plans of care within state-specific guidelines.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
