Job Summary:

As a Licensed Practical Nurse with Catholic Health Service, you will work under the direction of a Registered Professional Nurse, providing selected nursing care for sub-acute, convalescent, and chronic patients in their homes. This role requires a strong commitment to delivering high-quality patient care and a passion for working in a dynamic, fast-paced environment.

Responsibilities:

  • Perform selected nursing care for patients in their homes, under the direction of a Registered Professional Nurse.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle habits.

Requirements:

  • Graduate from an accredited school of nursing required.
  • Current NYS licensure as an LPN required.
  • Minimum of one (1) year combined direct patient care experience as an LPN required.

Preferred Qualifications:

  • Knowledge of rules and regulations governing Certified Home Health Agencies.
  • Strong written and verbal communication skills.
  • Basic Microsoft Office skill set.
  • Valid driver's license.

Working Conditions:

  • May be exposed to unsanitary conditions in some home settings.
  • May be exposed to high crime areas within the community or within the county by the agency.
  • Endure exposure to weather and temperature extremes.
