Project Manager, Rural Population Health

Job Summary:

The Rural Population Health Project Manager is a key member of MercyOne's Population Health Services Organization's (PHSO) Operations Team. This role is dedicated to supporting rural affiliates and partners in value-based contracts and population health management.

Key Responsibilities:

  • Serve as the primary liaison, educator, coach, and key source of communication for all key stakeholders.
  • Manage relationships between rural affiliates, the PHSO, and contracted third parties in relation to population health.
  • Organize statewide governance meetings focused on performance reporting, identifying improvement opportunities, and joint decision-making among all partners.
  • Effectively facilitate team meetings and leadership alignment at all levels to achieve performance aims, program goals, and deliverables.
  • Prepare payer performance reporting and patient/care gap lists for each organization on a monthly basis.
  • Work with rural organizations, data analytics teams, and IS affinity groups on data integration as part of value-based contract requirements.
  • Implement performance, quality improvement, and other operational initiatives/programs statewide.
  • Use critical thinking and analytical skills to determine needed education, training, or interventions to overcome barriers.

Requirements:

  • 3 years of previous project management experience required.
  • Bachelor's degree required. Proficient use of databases and Microsoft Excel spreadsheet applications required.
  • Experience in project outcomes evaluation and data analysis required.
  • Knowledge of rural health care processes a strong asset.
  • Experience in quality/process improvement initiatives preferred.

Work Environment:

This role involves extensive reading, preparing, and analyzing data. Adaptability to accepting responsibility for the direction, control, or planning of an activity is essential. The ability to maintain a high standard of courtesy and cooperation in dealing with co-workers, vendors, providers, and stakeholders is also required.
