About the Role

We are seeking a highly skilled and experienced Registered Nurse to join our dynamic team as a Team RN. As a key member of our care team, you will play a vital role in delivering exceptional patient care and contributing to the success of our organization.

Key Responsibilities
  • Provide leadership to the care team to ensure efficient patient flow and completion of daily tasks.
  • Assign care team members in accordance with patient needs and staff capabilities and qualifications.
  • Participate in performance management of the care team support staff.
  • Provide population health management for empaneled patients to assist with chronic care management and addressing gaps in care.
  • Perform quality assurance reviews for life safety controls/tasks and duties assigned to clinical support staff.
  • Triage patients in-person or on the phone according to established protocol.
  • Coordinate patient medication management, ensuring comprehensive medication reconciliation, appropriate patient education, and completion of medication refills per standing orders.
  • Document patient care accurately and timely in the electronic health record.
  • Assist clinical support staff as needed, including facilitating patient rooming and intake, obtaining vital signs, performing point-of-care testing, administering medications, and completing prior authorizations and referrals.
Requirements
  • Completion of an accredited RN program accepted by the Iowa Board of Nursing for licensure.
  • A minimum of 2 years of RN experience. LPN experience may be considered to meet the minimum experience requirement for internal candidates at leadership discretion.
  • Excellent customer service skills.
  • Effective English communication skills, written and verbal.
  • Knowledgeable of nursing care regulations and practices established by the Iowa Board of Nursing.
  • Demonstrates competency of required nursing skills.
  • Contributes to a team approach to care.
  • Effective leadership skills.
  • Accurate and efficient typing skills.
  • Efficiently performs basic computer functions.
  • Demonstrates competency using electronic health record.
  • Demonstrates attention to detail in work.
  • Prioritizes assigned work.
  • Establishes and maintains effective working relationships with a diverse group of individuals.
  • Interacts with others in an empathetic manner while maintaining professional boundaries.
  • Remains calm and works through difficult interactions.
  • Effectively resolves problems.
  • Utilizes universal precautions in patient care.
