Advanced Heart Failure Nurse Practitioner

We are seeking a highly skilled Advanced Heart Failure Nurse Practitioner to join our team at Hackensack Meridian Health. As a key member of our healthcare team, you will provide exceptional patient care and work collaboratively with other healthcare professionals to deliver high-quality patient outcomes.
Key Responsibilities- Provide medical assessments, treatments, and clearances for patients and employees in accordance with company profiles and regulatory standards.
- Develop and implement appropriate plans of care, ensuring accurate and timely documentation of patient information.
- Collaborate with physicians and other healthcare professionals to ensure seamless patient care and effective communication.
- Prescribe medications and recommend over-the-counter medications as necessary, ensuring safe and effective patient care.
- Initiate laboratory tests, immunizations, and diagnostic tests/procedures as appropriate, adhering to regulatory and scope of practice guidelines.
- Participate in continuing education and maintain documentation of professional development, ensuring up-to-date knowledge and skills.
- Provide culturally diverse and educationally appropriate education to patients and families, promoting health literacy and patient empowerment.
- Float to various clinic locations as needed, demonstrating flexibility and adaptability in a fast-paced healthcare environment.
- Master's degree in nursing from an NLN/AACN accredited program.
- Current NJ State Professional Registered Nurse License and Advanced Practice Nurse Certification.
- AHA Basic Health Care Life Support HCP Certification.
- Minimum of 3 years of professional experience in a relevant patient care setting.
- Strong organizational and computer skills, with effective verbal and written communication skills.
- Emotional intelligence, critical thinking, and effective interpersonal skills, with the ability to adapt to multiple and changing priorities.
