We are seeking a skilled Palliative Care Nurse Practitioner to join our team at Hackensack Meridian Health. As a Palliative Care Nurse Practitioner, you will play a vital role in providing high-quality patient care and support to individuals with complex health needs.
Key Responsibilities:- Provide medical assessments, treatments, and clearances for patients and staff members.
- Develop and implement appropriate plans of care, taking into account the patient's medical history, current condition, and treatment goals.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for patients and their families.
- Prescribe medications and recommend over-the-counter medications as needed.
- Initiate laboratory tests, immunizations, and diagnostic tests/procedures as appropriate.
- Document patient findings, recommendations, and education in a clear and systematic manner.
- Graduate of an NLN/AACN accredited program in nursing.
- Masters Degree in Nursing.
- Advanced Practice Nurse Certification (with Master's Degree).
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification.
- Masters Prepared Advanced Practice Nurse.
- Minimum of 3 years professional experience.
- Strong organizational and computer skills.
- Effective verbal and written communication skills.
- Emotional intelligence, critical thinking, and effective interpersonal skills.
