Rehabilitation Services Coordinator

**Job Summary:"
As a Rehabilitation Services Coordinator - Case Management Specialist, you will be responsible for providing comprehensive case management services to clients in Jefferson County, Hardin County, and the Golden Triangle location. You will work closely with our team of rehab clinicians to ensure that clients receive the support and resources they need to achieve their goals.
**Responsibilities:**
* Conduct clinical screenings, assessments, and service eligibility determinations to identify client needs and develop individualized service plans.
* Collaborate with rehab clinicians, family members, and community partners to ensure that clients receive comprehensive support services, including skill-building training, medication training, and crisis intervention.
* Maintain accurate and up-to-date client records, including progress notes, service plans, and outcome measures.
* Participate in staff meetings, monthly training, and professional development activities to stay current on best practices and industry developments.
* Collaborate with court/probation, tribal/community agencies, families, and other natural support systems to ensure clients' needs are met.
**Requirements:**
* Master's degree in a related field, such as social work or counseling.
* At least one year of paid experience in case management or a related field.
**Salary:**
$55,000 - $65,000 per year, depending on experience.
