Job Description

This role supports the North Hartford Ascend Pipeline (Ascend), a place-based initiative that integrates schools and community services to help children and families reach their full potential.

The Program Coordinator will:

  • Support a coordinated pipeline of services

  • Support early identification, referral, and linkage to community and social services and supports
  • Support data collection and analysis to monitor implementation and impact of Ascend

Ascend is funded by a five-year grant from the U.S. Department of Education Promise Neighborhoods program and sponsored by Connecticut Children's.

Under general supervision, the Program Coordinator will coordinate and perform a variety of program implementation duties for the Injury Prevention Center and Healthy Homes program, and directly support the North Hartford Ascend Pipeline program.

The incumbent will support the implementation of community-based safety programs for children, young adults, and families, and connect qualifying families to remediation services and resources, supporting families through the remediation process.

Responsibilities

  • Assist in development, implementation, and evaluation of community-based child unintentional injury prevention programs, specifically the Injury Free Coalition for Kids and the North Hartford Ascend Pipeline
  • Create, organize, and disseminate injury prevention materials, workshops, presentations, and training through organizations, schools, community groups in the prioritized communities
  • Respond to requests for information regarding home safety from parents, organizations, and coalition members, including organization and implementation of home safety audits
  • Collaborate with coalition members and key organizations to assist in coordination of injury prevention activities and information
  • Collect data for research and evaluation
  • Assist with other communications and activities relating to the completion of project objectives
  • Provides in-home education and follow-up services to families referred through NHAP partners and agencies
  • Adapts procedures and processes to meet requirements of the program
  • Perform various duties in support of the Program Manager in support of achieving grant benchmarks
  • Support the activities of the IPC, including ensuring safe discharge from the hospital as needed

Under the direction of IPC leadership, the Program Coordinator will perform a variety of assignments including the preparation, editing, and distribution of routine to complex documents & correspondence; may create forms, flyers, templates as needed.

Maintains and expands own competencies by attending training sessions or educational programs, as directed.

Demonstrates knowledge of the population-specific differences and needs of patients and community members in appropriate, specific populations from young children through adolescence and applies them to practice.

Demonstrates cultural awareness in all interactions with patients/families and co-workers.

Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the Connecticut Children's Standards.

Qualifications

  • High School Diploma, GED, or a higher level of education that would require the completion of high school, is required.
  • Associates degree or administrative certificate preferred, with 3 to 5 years progressively increasing experiences directly related to the duties and responsibilities specified

Knowledge of:

  • Strong working knowledge of Microsoft Windows operating system
  • Working knowledge of scheduling software such as MS Outlook, or other software may be required by individual departments
  • Departmental and organizational policies and procedures
  • General math and accounting procedures

Skills:

  • Intermediate to Advanced level word processing skills and spreadsheet skills
  • Database management skills including ability to utilize database applications, perform basic queries and produce reports
  • Skill in use of operating basic office equipment (e.g. copier, fax, calculator, multi-featured telephone)
  • Effective use of patience in day-to-day performance

Ability to:

  • Understand and apply knowledge of department/organizational policies and procedures to resolve routine to complex problems and meet multiple needs of customers
  • Multi-task and problem solve with working knowledge of unit/department
  • Organize and manage time, activities and processes on weekly and/or project basis
  • Make administrative/procedural decisions and judgments
  • Focus on customer needs and follow through to meet needs
  • As assigned, gather, review and compile information and prepare accurate and comprehensive reports, often with deadlines
  • Maintain calendars and schedule appointments utilizing electronic scheduling software
  • Create, compose and edit written materials
  • Function in a fast paced environment with strong attention to detail
