Job Summary

We are seeking a highly motivated and compassionate individual to join our team as a Supportive Housing Program Coordinator. In this role, you will have the opportunity to make a meaningful difference in the lives of individuals with intellectual and developmental disabilities.

Key Responsibilities
  • Lead and transport residents to group activities, promoting socialization and community integration.
  • Participate in and assist residents with activities of daily living, ensuring their physical and emotional well-being.
  • Act as a liaison to residents and their families or community agencies and organizations, providing support and guidance as needed.
  • Implement programs and provide care, guidance, training, and supervision to residents living in the community.
  • Work independently over an assigned geographic area, providing individualized family support, in-home and community support, and respite services.
  • Assist residents and families in securing resources for ongoing support needs, promoting independence and self-sufficiency.
  • Provide transportation and assistance to help residents access community resources, attend medical and dental appointments, and engage in other activities.
  • Document treatment and care plans, ensuring accurate and timely reporting to Case Managers.
  • Assist residents transitioning from their family home to their own homes, providing support and guidance throughout the process.
  • May assist with eligibility for DDS visits, assessing situations and completing forms as needed.
  • Provide follow-up assistance for residents with AID issues, ensuring access to food, transportation, and other essential resources.
  • Directly support residents who need assistance participating in their community, promoting socialization and integration.
  • Assess life skill abilities for residents, identifying areas for improvement and developing strategies for growth.
  • Function as a member of the planning and support team, collaborating with colleagues to ensure comprehensive support services.
  • Provide concise and accurate computer-based reports and documentation, ensuring timely and effective communication with stakeholders.
  • May administer and track medications with appropriate certification, ensuring resident safety and well-being.
  • May administer CPR first aid or PMT in emergency situations, responding promptly and effectively to critical situations.
Requirements
  • Knowledge of modern methods of working with residents with intellectual disabilities and other related conditions.
  • Knowledge of safety, hygiene, and health practices and procedures.
  • Knowledge of state and federal assistance programs.
  • Knowledge of relevant state and federal laws, statutes, regulations, and procedures.
  • Some knowledge of medication administration and effects of medication.
  • Considerable interpersonal skills.
  • Oral and written communication skills.
  • Ability to recognize and report behavioral changes.
  • Ability to coordinate and collaborate effectively with residents, groups, and families.
  • Ability to work independently and make decisions.
Preferred Qualifications
  • Experience working with adults with intellectual disabilities and/or autism spectrum disorders.
  • Experience working with residents with intellectual disabilities who live independently or at homes.
  • Experience working on multiple projects at a time while balancing competing deadlines.
  • Experience working with children and adults who present with medical complexities and behavioral challenges.
  • Experience communicating with parents and caregivers.
  • Experience working as a member of an interdisciplinary team, specifically with clinicians and case managers.
  • Experience with community resources for residents with disabilities.
  • Experience teaching residential life skills and assessing those skills with residents who have intellectual disabilities.
  • May be required to possess and retain a current Connecticut Motor Vehicle Operator license.
  • May be required to be certified in medication administration.
  • Incumbents in this class may be required to travel.
