About the Role

We are seeking a highly skilled and experienced Licensed Clinical Therapist to join our team as a Behavioral Health Specialist. In this role, you will work closely with our primary care team to provide comprehensive behavioral health services to patients in a variety of settings.

Key Responsibilities
  • Provide brief, evidence-based interventions to patients with physical and mental health or substance use conditions
  • Collaborate with primary care providers to develop and implement treatment plans
  • Conduct assessments and screenings to identify patients' behavioral health needs
  • Develop and implement educational materials and presentations on mental health and substance use topics
  • Participate in team-based care and contribute to the development of a culture of innovation and professionalism
Requirements
  • Master's degree in Mental Health Discipline
  • Licensed Clinical Social Worker and Licensed Mental Health Counselor (LCSW or LMHC) in the State of Florida
  • 3+ years of experience working with older adults and patients with SMI
  • 3+ years of experience delivering brief, evidence-based interventions
  • Experience with team-based care
  • Experience with Motivational Interviewing
  • Knowledge of and experience with several screening tools such as PhQ9, GAD 7, CSSRS, AUDIT, SBIRT, & CAGE
Preferred Qualifications
  • Dually licensed as a mental health and substance use clinician
  • 1+ years of experience in primary care or related settings
  • Experience facilitating psychotherapy, process, and psycho-educational groups
  • Experience working with medical provider(s) performing Shared Medical/Behavioral Health Appointments
  • Experience creating educational materials for the patient and care team and presentations on mental health and substance use topics
  • Experience with crisis intervention
  • Experience with Geriatrics or Certified Geriatric Specialist
  • Experience with health psychology and behavioral medicine
  • Knowledgeable of hierarchical condition category (HCC) coding
