Palliative Care Nurse Practitioner

We are seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our team at Providence Medical Group in Western Montana. As a Palliative Care Nurse Practitioner, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Provide high-quality, patient-centered care to individuals with serious illnesses
- Collaborate with a multidisciplinary team to develop and implement individualized care plans
- Conduct thorough assessments and develop treatment plans to address the physical, emotional, and spiritual needs of patients
- Work closely with patients, families, and healthcare providers to ensure seamless care transitions
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
This is a 0.75 FTE position, working 3 ten-hour days per week. You will have a flexible schedule and no weekends or call. We are seeking a candidate with at least 2 years of Palliative Care experience, but we are open to candidates with other relevant experience.
