About the Role

Responsibilities

  • Collaborate with healthcare teams to develop and implement comprehensive care plans
  • Conduct thorough assessments, develop and implement treatment plans, and provide ongoing care and support to patients and their families
  • Stay up-to-date with the latest evidence-based practices and guidelines in hospice and palliative care
