About the Role

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at Providence Medical Group - Western Montana. As a key member of our interdisciplinary team, you will provide compassionate and high-quality care to patients and their families in a dynamic and supportive environment.

Responsibilities
  • Provide comprehensive palliative care to patients with serious illnesses, focusing on symptom management, pain control, and quality of life.
  • Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and chaplains, to develop and implement individualized care plans.
  • Conduct thorough assessments, develop and implement treatment plans, and provide education to patients and their families.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures related to palliative care.
Requirements

To be successful in this role, you will need:

  • 2 years of experience as a Palliative Care Nurse Practitioner.
  • Advanced practice registered nurse (APRN) licensure and certification in palliative care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • A commitment to providing high-quality, patient-centered care.
