Mental Health Care Coordinator

Marion County is seeking a highly skilled and compassionate Exceptional Needs Care Coordinator to join our team. As a key member of our Adult Behavioral Health team, you will provide critical support to individuals with serious and persistent mental illnesses and complex needs.
Job Summary:The Exceptional Needs Care Coordinator will work closely with community and state partners to assess individuals' needs, determine eligibility for services, and authorize enrollment for Choice Model services. This role requires strong clinical skills, excellent communication, and the ability to work collaboratively with a multidisciplinary treatment team.
Key Responsibilities:- Act as a liaison between ABH and various stakeholders involved in mental health care, including Oregon State Hospital, acute care hospitals, and coordinated care organizations.
- Collaborate with individuals, partners, and stakeholders to assess needs, determine eligibility, and authorize services.
- Attend OSH Interdisciplinary Treatment Team meetings and participate in Ready to Transition determinations.
- Facilitate community transitions for individuals who meet Choice criteria.
- Provide case-specific consultation to providers and community partners.
- Ensure accuracy of utilization data and provide routine reports to Marion County Management.
- Master's degree in psychology, social work, or a related field.
- One year of postgraduate degree experience in the behavioral health field.
- Must be licensed by the Oregon Board of Licensed Professional Counselors, licensed by the Oregon Board of Licensed Social Workers, or credentialed/registered through the Mental Health and Addiction Counseling Board of Oregon as a Qualified Mental Health Professional (QMHP).
- LCSW, CSWA, or eligible for LCSW licensure.
This is a safety-sensitive/special needs position. Final candidates will be required to pass a pre-employment drug screen and a criminal history background check.
