Community Access Unlimited (CAU) is a social service agency that provides support to individuals with intellectual and/or developmental disabilities, as well as youth within our community.

Job Summary:

We are seeking an individual to assist in the creation and implementation of agency Support Plan protocol and review processes, attend Support Plan meetings, and monitor, track, and review IHP and ISP for individuals with developmental disabilities and/or Transitional Plans for youth success.

Responsibilities:
  • Monitor, track, and review assigned Annual Individual Habilitation Plans (IHP), Individual Service Plans (ISP), and/or Transitional plans for youth success
  • Develop training materials and ensure all are up to date with licensing standards
  • Review members' documents, annual support plans, modification forms, health and safety documents, medical information, and other documents or information pertaining to the quality of services provided to the members
  • Ensure the agency is in full compliance with all federal, state, and local licensing regulations
  • Attend Support Plan meetings
  • Complete audits and on-site inspections
  • Ensure that required documentation is complete and is in compliance with Agency and DDD regulations and standards
Qualifications:
  • Experience with ISP, NJ State Licensing requirements, and/or DCP&P transitional plans for youth success
  • Knowledge of DCP&P regulations
  • Knowledge of DDD regulations
  • Excellent computer and Microsoft Office skills
  • Excellent communication skills, verbal and written
  • Highly organized and self-motivated
  • High School diploma/GED
  • Valid driver's license
  • Access to a car to use for work purposes
  • Willing and able to attend required trainings outside of normal work hours
Experience:
  • Working with individuals with developmental disabilities: 1 year (Preferred)
  • Individualized Support Plans: 1 year (Required)
  • DDD Licensing: 1 year (Required)
  • DCP&P Transitional Plans: 1 year (Preferred)
